HELPFUL HINTS FOR TURNING IN YOUR PAPERWORK AT WALK IN REGISTRAION AND TIPS ON SENDING IN YOUR REPORT CARD AND PHYSICAL BY JUNE 30TH.
When attending one of the in-person registrations, you will need:
1. To bring your daughter
2. Participant contract (please be thorough, and daughter and parent sign page 2).
3. Bring a copy of the Participant Contract you just completed.
4. Make a copy of their birth certificate OR passport. Please do not bring originals. Note: Hospital "certificates" are not accepted. (i.e., keepsake with the tiny feet on it:) It must be a copy of the certificate that was filed with the county or state.
Remember, returning participants must attend one of the first two in-person registrations to maintain their priority status.

By June 30th, we need:
1. A complete copy of your year end report card. Both semesters for Middle School.
2. A physical on our TPPW form filled out by your Dr./Nurse
3. The medical history form completed by a parent
4. A copy of the physical and medical history form you just completed
HELPFUL HINTS!
REPORT CARD TIPS:
1. On the last day of school, remember to get your child's report card out of their back pack and put it in a safe place. If it's mailed from the school, keep an eye out for it.
2. When you make a copy, be sure that ALL info is copied and not cut of. Many times the name, school name or grade level is cut off and we don't want to pester you for another copy.
3. If the report card is legal size, please reduce it from Legal to Letter size, or reduce by 78% so it fits in our team book.
PHYSICAL AND MEDICAL HISTORY FORM TIPS:
1. A physical needs to be completed on our TPPW form by your Dr. If your child has had a physical as of 1/1/10 their office can transfer the information to our form.
2. You can get a "walk in" sports physical at PPH Expresscare, located in Albertsons/Sav-on at 14340 Penasquitos Dr. SD 92129. Last year the cost was $25.
3. Please be sure that the full address and phone number of the practitioner is on the physical form. Many times not all info is on their address stamp.
MAILING TIPS FOR YOUR REPORT CARD AND PHYSICAL/MEDICAL FORMS:
1. Remember to make a copy of what you mail in. Things do get lost, and this will save you some frustration if your paperwork doesn't make it to us.
2. MARK THE OUTSIDE OF THE ENVELOPE ATTN: CHEER
3. DO NOT SEND FOOTBALL AND CHEER documentation together for siblings, as they go to two different divisions of TPPW. Send football in a separate envelope attn: Football.
3. You can mail or hand carry your envelope and drop off at our Post Mail Box location, located at Postal Annex (between Vons and Baskin Robbins)TPPW-PMB 551, 3830 Valley Center Dr. #705, SD 92130.
