Remember, the physical must be completed on our Pop Warner form: Medical and Physical Form 2010
If your doctor has performed a physical, as of January 1, 2010, you can have that information transferred to our form. You do not need a 2nd physical.
You can also get a "walk in" sports physical at PPH Expresscare, located in Albertsons/Sav-on at 14340 Penasquitos Dr. SD 92129. Last year the cost was $25.

Appointments are not necessary and most visits take about 20 minutes. For your convenience, pagers are available so you may shop, visit the snack area, complete your banking and/or Albertson's customer service needs until the Nurse Practitioner is available to see you.

PPH expresscare is closed Monday-Friday from 1:30-2:30 PM and Saturdays and Sundays from 1:30-2:00 PM for required lunch breaks. The last patients are accepted 30 minutes before closing time each evening.
